Select Boxes shows a list of options as check boxes, so users can choose any number of them. Use it for questions such as "Which of these apply?".
For one choice only, use Radio. For a single yes-or-no box, use Check Box.
Set up the options
On the Display tab, under Values, add a row for each option with a Label (shown to users) and a Value (saved). No option has to be selected unless you turn on Required.
How the answer is saved
The value is a set of true / false answers keyed by each option's value. Only boxes the user has touched are included: a box that was never checked may be missing rather than false. For a component symptoms with values fever, cough and headache, a user who checks only the first two saves:
{ "fever": true, "cough": true }
-
In a script, test one option with
data.symptoms && data.symptoms.fever. A missing option counts as not checked. -
In a simple condition, set Has the value to the option's value, for example
fever. The condition matches when that box is checked. -
When mapping to a PDF or another system, treat a missing option the same as unchecked.
