General Forms

Document Conversion — Using a simple form process, users can easily scan or attach a document and convert it to a variety of editable or archive document formats. This example allows the user to choose what type of document to create from their Tiff, PDF, or JPG attachment.
Landing Pages — Forms can be tagged, allowing them to be grouped together and displayed in a common landing page. This allows form builders to quickly and easily add links to form "dashboards", giving users the ability to access all forms for a particular task from a single location. Change the "tag" parameter in the URL to demonstrate different form collections.
Freedom of Information (FOIA) Request — This is a great example of how a basic form can greatly simplify traditional paper processes. A form like this is in use by most municipalities to log public records requests. In just a few minutes, a customer can build a form like this and automatically file requests for information to ensure audit and compliance standards.
Patient Screening — The patient screening form demonstrates a guided form process that takes the user through a series of standard questions leading up to an MRI. Multiple pages are used to guide the user through the necessary steps, including the ability to mark up the location of metal objects in the body use mouse, stylus, or finger. Be sure to provide an email address to receive a copy of the complete form.
