Patient Screening Form
The patient screening form demonstrates a guided form process that takes the user through a series of standard questions leading up to an MRI. Multiple pages are used to guide the user through the necessary steps, including the ability to mark up the location of metal objects in the body use mouse, stylus, or finger.
